Knowing how your insurance company will pay for the services you receive and whether the facility is a participating provider will help avoid unexpected costs later. It is each patient's responsibility to know their insurance contract and which providers participate with their insurance plan. The pricing information provided by the website below is a non-personalized estimate of costs that may be incurred by a patient for anticipated services and that actual costs will be based on actual services rendered. The service bundle pricing offered through this weblink is based on the average healthcare costs for both local and national pricing.
Patients may request a personalized estimate by calling Apollo Surgery Center at (321) 726-9393 ext. 314.
To receive an estimate, you will need to provide:
- Complete contact information (name, address, e-mail and valid phone number)
- Health insurance information
- The CPT code(s) for the procedure(s). Your surgeon’s office can provide the CPT code(s) to you. This is needed to ensure the estimate is given for the correct procedure(s).
Your personalized estimate will be provided within 7 days of your request. We will let you know if we participate with your insurance plan.
Important note: Apollo Surgery Center provides an estimate only. The actual care provided to you will determine your final bill. Estimates cover only Apollo Surgery Center facility charges and do not include other charges such as surgeon professional fees, anesthesia services and pathology services.
Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider. These other health care providers separately bill the patient and may or may not participate with the same health insurers or health maintenance organizations as the facility. You will need to contact the resources below in order to know their participation with your insurance company, billing practices and to receive your personalized estimate.

Payment Policies
Apollo Surgery Center is a for-profit facility and does not have a charity care provision or offer financial assistance beyond Care Credit and payment plans which have been discussed prior to surgery.
Payment for self-pay, copay, co-insurance and deductible is due on date of service based upon your benefits and estimated insurance reimbursement. We accept cash, money orders, cashier’s checks and all major credit cards including Care Credit and Alphaeon.
Personal checks are not accepted.
Collections procedures: A statement of account will be sent every month for three months. After 90 days, a final statement will be sent allowing 10 days to pay. If no payment is received after the 10 days have passed, the account may be sent to collections.
