Knowing how your insurance company will pay for the services you receive and whether the facility is a participating provider will help avoid unexpected costs later. It is each patient's responsibility to know their insurance contract and which providers participate with their insurance plan. The pricing information provided by the website below is a non-personalized estimate of costs that may be incurred by a patient for anticipated services and that actual costs will be based on actual services rendered. Patients may request a personalized estimate by calling Apollo Surgery Center at (321) 726-9393 ext. 314.
To receive an estimate, you will need to provide:
- Complete contact information (name, address, e-mail and valid phone number)
- Health insurance information
- The CPT code(s) for the procedure(s). Your surgeon’s office can provide the CPT code(s) to you. This is needed to ensure the estimate is given for the correct procedure(s).
Your personalized estimate will be provided within 7 days of your request.
Important note: Apollo Surgery Center provides an estimate only. The actual care provided to you will determine your final bill. Estimates cover only Apollo Surgery Center facility charges and do not include other charges such as surgeon professional fees, anesthesia services and pathology services.
Payment Policies
Payment is due on date of service based upon your benefits and estimated insurance reimbursement. We accept cash, money orders, cashier’s checks and all major credit cards. Personal checks are not accepted.
